Case Manager
Des Moines County is seeking a motivated professional to work within our Targeted Case Management Program. The candidate will serve individuals who have a diagnosis of Mental Retardation, Chronic Mental Illness, Developmental Disability or Brain Injury. We are looking for a team player who is detail-oriented, flexible, a good communicator and an effective problem solver. Proficiency in computer applications is required. An excellent benefit package is offered.
Education/Experience
Requirements are:
1) A bachelor's degree in behavior sciences, education, health care, human services or social science (at least 30 semester hours of college credit in these areas ) & one year of delivery of services to persons diagnosed as MR/CMI/DD
OR
2) An Iowa license to practice as a registered nurse and at least 3 years of direct delivery of services to each targeted population.
