The case manager will establish a positive relationship with the consumer providing guidance, advocacy and effective communication in providing information to the individual to participate to their greatest extent in the decision making process. The TCM will involve the individual, family, guardian and other professionals in identifying, developing, implementing and monitoring a comprehensive plan. The TCM will strive for the individual to attain a greater level of integration into the community, stabilize and to graduate from case management services." Case Management is a service offered to people with diagnosis of Chronic Mental Illness, Mental Retardation or Developmental Disabilities. We assist recipients in gaining access to appropriate living environments, needed medical services and interrelated social, vocational, and educational services. Case management services link consumers to service agencies and support systems responsible for providing the necessary direct service activities and coordinate and monitor those services. This program is consumer directed and assists the individual in living and working in the least restrictive environment in the community. Eligibility criteria include Title XIX eligibility, a documented diagnosis of CMI, MR, or DD. It is a voluntary program for adults. Children under 18 years of age, must be recipients of the HCBS/MR waiver. Individuals may be referred by other agencies or apply directly to the Des Moines County Community Services office, 910 Cottonwood, Ste 1000, Burlington, IA. If needed, a case manager may meet with the individual or guardian in their home to determine eligibility and appropriateness of the service. Top of PageGeneral AssistanceGeneral Assistance Worker: Alana Capps Des Moines County Community Services, in accordance with Iowa Code, chapter 252 and Chapter 255, provides assistance to the needy and poor people in need of immediate assistance or people who cannot obtain assistance from any other source. Assistance is provided to an individual that has income or benefits delayed from a state program or not actually received because of reasons not attributable to the person, and who does not have liquid assets to pay for the items of assistance that can be provided. There are different categories of individuals served in Des Moines County.
Community Services provides assistance to people who need help with rent, utilities, limited medical care and prescriptions and burial assistance. The program is designed to provide short-term emergency type assistance to persons lacking the resources to meet their basic needs for food, energy, shelter, and medical care. It is not intended to provide on-going, long term aid to any individual or family. Persons are also referred to other agencies for assistance with food and clothing. To apply, an individual should call the office at (319)754-8556 to schedule an appointment with Alana. They will be asked to bring documentation of income, identification, bills and rent receipts.
