Consumers may apply for the following services:
- Adult Day Care (MR only)
- Adult Development Services (MR only)
- Case Managment (CMI, MR, DD)
- Consultation
- CPC Administrative Costs (CPC Administrator & office)
- CPC Support Staff Costs
- Day Treatment
- Evaluations (including those for commitment)
- Group home (with psychosocial Rehabilitation)
- ICF-MR
- Nursing Services
- Inpatient/Hospital
- Legal Representation
- Mental Health Institutions
- Partial Hospitalization
- Patient Advocacy
- Payee Services
- Prescription Medicine
- Psychiatric Rehabilitation
- RCF (includes PMI and PMR)
- RCF-MR
- Rent Subsidy
- Sheltered Workshop Services
- State Hospital Schools
- Supported Community Living
- Supported Employment Services
- Transportation
- Work Activity Services
- HCBS
- Outpatient Psychotherapeutic Treatment - which only includes:
- Psychiatric Evaluations
- Psychiatric Services
- Psychological Testing
- Medication Monitoring and Management
- Family Counseling (includes couples counseling)
- Individual Therapy
- Group Therapy
- Supported Community Living
- Emergency Services
- Payee Services
With this wide array of services and supports, the County will be able to actively
pursue individualized services and support plans for those persons
making application under the Disability Services Management Plan.
Should the situation arise in which a consumer needs services of supports
which are not yet available, the county will actively pursue implementation
of those services and supports when financially feasible so that the
County may remain in compliance with the budget limits imposed by
legislation.
